Managing Major Depressive Disorder

Managing Major Depressive Disorder

Major depressive disorder (MDD) or Major Affective Disorder is a severe psychiatric disorder affecting over 200 million people worldwide. The lifetime depression risk is 15–18% and is higher in women than men.1 Aetiologically, while societal and environmental factors play a significant role in the psychosocial development of depression, underlying biological and genetic factors are also key drivers, and the two interact via epigenetic mechanisms.

Diagnosing and Treating Metabolic Syndrome

Diagnosing and Treating Metabolic Syndrome

The presence of the metabolic syndrome raises potential health concerns, particularly as we search for early disease markers with the aim of prevention and/ or early intervention. The metabolic syndrome refers to the co-occurrence of several known cardiovascular risk factors, including insulin resistance, obesity, atherogenic dyslipidaemia and hypertension. Management of metabolic syndrome and its comorbidities ideally involves a multidisciplinary team approach to risk-factor management.

Timely Diagnosis of Pre-eclampsia is Key

Timely Diagnosis of Pre-eclampsia is Key

Pre-eclampsia ( P E ) i s a pregnancy-specific multisystem hypertensive disorder that is a major contributor to maternal, neonatal morbidity and mortality. Angiogenesis-related factors, including sFlt-1 (soluble fmslike tyrosine kinase 1) and PlGF (placental growth factor), play an important role in placental dysfunction; altered levels are detectable several weeks before onset of pregnancy complications. In vitro diagnostic tests for these biomarkers can improve early diagnosis and facilitate prediction of maternal and foetal outcomes.

Diabetes in Older People, How does management change?

Diabetes in Older People, How does management change?

Diabetes in older adults is a growing health burden in South Africa and presents various challenges. Clinical manifestations of diabetes in older people are atypical and the heterogeneity of older people, ranging from the robust to the frail, should be strongly considered when developing treatment goals and strategies. Diabetes care in older people generally has a greater emphasis on maintaining quality of life, delivering personalised care, achieving comfort, and preventing symptoms and acute complications of hyperglycaemia.

Folliculitis: Diagnosis and management of subtypes

Folliculitis is a common disorder, although the exact prevalence is unknown. It presents as erythematous pustules surrounding hair follicles. It may be due to a superficial or deep infection of the follicle, or may be secondary to trauma. Deeper infections present as sycosis and folliculitis decalvans. Good grooming and hygiene are key to treatment and long-term resolution, alongside pharmacological therapies.

Topical Help for Clearing Rosacea, a Common Skin Disorder

Recognised as one of the most common and clinically characteristic facial skin disorders, rosacea is an inflammatory dermatosis with a reported prevalence of at least 10% among white adults. Ivermectin 1% cream has been shown to be an effective, well-tolerated, and safe treatment for papulopustular rosacea in adults in several randomised, controlled trials of subjects with moderate-to-severe disease.

Putting the Brakes on Asthma Airway Inflammation in Children

Putting the Brakes on Asthma Airway Inflammation in Children

Asthma is the most common chronic disease of childhood and its prevalence has substantially increased worldwide, particularly in pre-school children. Chronic inflammation and smooth muscle dysfunction are consistent features of asthma pathophysiology, responsible for disease progression and airway remodelling. Montelukast, a cysteinyl leukotriene 1 receptor antagonist, is indicated as an alternative treatment in paediatric asthma management and to relieve symptoms of seasonal allergies.

Patient Care Hampered by the Interprofessional Divide

Patient Care Hampered by the Interprofessional Divide

A central aim of modern day healthcare is to deliver a high quality, patient-centred service that addresses the expectations of its service users. However, mounting research evidence highlights a lack of patient satisfaction across a range of healthcare settings, with an overwhelming proportion of complaints relating to interprofessional communication.
